aTbRef update

I've just reviewed updated the Export Code section of aTbRef to try and
harmonise the words used as code argument descriptors. Thus all 'note'
entries are now 'item', e.g. ^aCode(note)^ -> ^aCode(item)^. The words used
in aTbRef don't necessarily match those in the TB manual. The arguments'
functions are the same, but more consistent terminology will hopefully help
those getting started with use of thee codes.

I've also put a generic listing describing these (and the ambiguities over
possible evaluations) on http://www.acrobatfaq.com/tbx/index/exportco.html

Do people find this useful? Any feedback, suggested changes, or errata
welcomed either directly or via this list.

Meanwhile, over on the wiki Sunir Shah's has just started adding some useful
structure to the existing content (the lack of an 'index' being a regular
observation from new wiki users). Expect the pages to alter some but you can
