Hi, Dave -

I create one note for each book, then use that note as a container
for individual notes with each quotation that I want to keep. When I
use the quote in an outline, I create an alias so I can easily follow-
back and get its source.

The metadata that I track is pretty much the same as yours, but I add
"publisher" and ISBN (when it's available.)
